    HOW TO APPLY FOR SADAREM CERTIFICATE IN TELANGANA
    SADAREM certificate is very important for the person’s suffering from Disability. This certificate help disabled persons' in accessing schemes of Government of central or state. SADARREM certificate is called disability certificate. Disability certificate is issued by the Local Medical Board. our Government provides so many benefits / reservation to disabled person like age concession while applying jobs in Government or private sector, fee concession in school / college fee, age limit while taking admission etc.

    For Disability Certificate, applicant needs to take an application form and then take an application form. Applicant can also apply through MeeSeva. The procedures for applying Disability Certificate are given below. Through these procedures you can apply easily for SADAREM certificate.

    PROCEDURES FOR REQUEST FOR SADAREM CERTIFICATE THROUGH MEESEVA
    Applicant can follow the below given procedures for Request for SADAREM certificate:

    First Step: First you need to take an application form. The application form for Request for SADAREM certificate, applicant needs visit to your meeseva department or you can visit to the official website of MeeSeva. The official website link of MeeSeva is given below:


    Second Step: On the page of Meeseva website, click on “MeeSeva Service Forms”. from the below given link , you can directly visit to the MeeSeva Service Forms page:


    Third Step: On next page, different departments name are to be given. For downloading application form of SADAREM certificate, applicant must click on the Rural Development department option. On this department, applicant got the application form.

    Fourth Step: From the Rural Development option, applicant needs to click on the form option of SADAREM certificate. And then download the application form and take a print out of the application form.

    Fifth Step: After downloading the application form of SADAREM certificate, fill all the necessary details in the application form. All the details to be filled in application form should be correct. Details should be filled in capital letters.

    Sixth Step: After filling all the details in the application form. Submit the application form to the MeeSeva . No document is required to attach with application form. And do not forget to take an acknowledge slip. After prescribed time MeeSeva provide you SADAREM certificate.

    (B) If choose yes, fill following details
    • Pension card number

    (C) If choose No, please fill below given details
    • Surname

    • Father / Husband / Mother / Guardian name relation
    • Gender
    • House no
    • Age
    • Phone number
    (D) Address
    • District
    • Mandal
    • Panchayat
    • Town / village
    • Habitation / ward number
    (E) Proof of documents
    • Proof type
    • Proof ID
    (F) Applicant signature

    FEE CHARGES
    There are two types of charges are to be charged by MeeSeva for providing every type of services is:
    Service charges: service charges for providing SADREM certificate MeeSeva charges fee as service charge are RS 25.

    Statutory Charges: No statutory charges are to be charged on the service of providing SADAREM certificate.

    Hi Sir,

    Please tell me incase if we have lost the PHC Certificate then what is the procedure to get it . My PHC certificate has been missed out by MRO office staff. Hence, please advice to get the certificate.

    Also, please advice whether we can download the certificate in internet, if yes please give the website name.
